How to Complete Your Intake Questionnaire

Your intake questionnaire is how your provider gets the information they need to evaluate you safely. It takes most patients under 10 minutes. Here's what to expect and how to set yourself up for a smooth review.

What you'll be asked:

  • Basic demographics and contact information

  • Current physical location (required β€” providers must be licensed in the state where you're receiving care)

  • Health history: past diagnoses, surgeries, hospitalizations

  • Current medications and supplements (list everything, including OTC)

  • Allergies

  • Relevant vitals or lab results if you have them

  • Goals and treatment preferences

  • Anything else you want your provider to know, or any questions you want answered

Be accurate and complete. Your provider can only work with what you give them. Incomplete or inaccurate information can result in a denied request or, worse, a prescription that isn't appropriate for your situation. This is a clinical review, not a formality.

You're also encouraged to ask questions. The last section of intake includes an open field specifically for anything you want to ask your provider or bring to their attention. Use it.

Common reasons for delays:

  • Missing medication list

  • Incomplete health history

  • Conflicting information between fields

If your provider needs clarification, they'll reach out via secure messaging inside your patient portal. Keep an eye on notifications.

Can I edit my intake after submitting?

You can't edit a submitted intake form directly, but you can message your provider through the portal to provide additional information or corrections. They'll factor it into their review.
